Clinical Nurse Educator (up to $150K/year)

Join our team as a Clinical Nurse Educator, earning up to $150K per year, and be responsible for developing educational programs for Clinical Teams within Home Health and Hospice. This role focuses on professional development, optimizing services and care, and is guided by the quadruple AIM—providing optimal care and experience for seniors and their families, applying a population health focus, being cost-conscious, and recognizing employee engagement as a key driver toward excellence.

 

Responsibilities:

• Design, implement, and evaluate clinical orientation programs, ensuring compliance with regulatory standards
• Conduct home visits to identify knowledge gaps, observe skill levels, and ensure adherence to best practices
• Develop and provide educational programs to enhance case management, clinical care, and performance
• Collaborate with Quality and Patient Safety teams to conduct individualized education sessions
• Create annual education plans, competency programs, and new protocols in partnership with other leaders
• Utilize data analytics to review outcome data, identify trends, and establish response plans

 

Qualifications:

• Active MA RN license with a minimum of 2 years of Home Care experience
• Strong communication skills and ability to make MS presentations
• Knowledge of Medicare Home Health regulations and acute care experience
• Clinical staff education experience and familiarity with Meditech or Epic systems
• Master’s in Public Health is a plus
• Ability to work collaboratively with team members and maintain high-quality educational activities
