Job Description

Harney District Hospital's Family Care Clinic is seeking a Community Health Worker (CHW) to join their team! This position is responsible for helping patients and their families navigate the healthcare system and community resources. The CHW will work closely with primary care teams and other agencies to ensure patients receive high-quality, comprehensive healthcare and are connected to available resources as needed. This position starts at $19.11 and increases based on experience. This is a full-time, on-site position. This position is also eligible to receive our full benefit package.

 

Required Skills and Abilities:

Must have a general familiarity with principles and practices in the medical field, i.e., importance of infection prevention, patient confidentiality, etc. Must have excellent time management, organizational, and work planning skills. Possesses working knowledge of community resources available to a rural population. Demonstrates cultural competency and the ability to work effectively with people of diverse backgrounds. Knowledge of word processing, spreadsheet and database application software.

Education and Experience

High school diploma or equivalent.

SPECIAL REQUIREMENTS/LICENSES: At time of hire or within 12 months of hire must be able to successfully complete CHW certification program. Must have a valid Oregon driver's license.

DESIRABLE REQUIREMENTS: Experience in healthcare or social work settings providing direct patient care and/or service. Highly independent, organized worker. Ability to inspire confidence in others. Demonstrated communication and customer service experience.
