Description The Community Health Worker role assists patients in accessing resources/services, supports disease self-management, and coordinates culturally-appropriate care and services.Required Minimum Knowledge, Skills, and Abilities (KSAs) * Education: High school diploma or GED preferred. * License/Certifications: If driving required for role, Valid Driver's license, clean driving record, and personal vehicle for use required. * Experience: Two years of experience working in a social service or health care setting or equivalent educational setting preferred. * Additional Skills/Requirements Required: Must be a resident of the community being served with good knowledge of the resources of their community. * Additional Skills/Requirements Preferred: Basic computer skills preferred.
