Mālama Ola means “to care for life and well-being.” We are currently seeking dedicated Community Health Workers to join our team and support our mission to deliver the highest quality care to those facing serious illnesses. Roles and Responsibilities: Interact with patients and families enrolled in hospice and palliative care, both over the telephone and in person. Meet with patients and families who are considering hospice and palliative care to provide information and support. Obtain and sign legal consents. Collaborate with our interdisciplinary team to ensure the best possible care for seriously ill patients. Perform data entry tasks to maintain accurate patient records and support program operations. Assist in the development and implementation of our care programs to enhance service delivery.
